Myn is recruiting on behalf of our long-term client in the South West of England. We are seeking a compassionate and dedicated Adult Social Worker to join the Community Mental Health Team (CMHT). This is an excellent opportunity to work with adults facing diverse challenges, including mental health issues, physical disabilities, and social difficulties.
About the RoleAs an Adult Social Worker, you will play a vital role in improving the lives of adults in the community. You will assess needs, develop personalised care plans, and coordinate with other professionals to ensure your clients well-being and quality of life.
Key Responsibilities- Assess Client Needs: Conduct thorough assessments to evaluate clients physical, emotional, and social needs.
- Develop Care Plans: Create tailored care plans that address individual goals, reviewing and adjusting them as required.
- Provide Support and Counselling: Offer guidance and emotional support to clients and their families, helping them navigate challenges.
- Coordinate Services: Collaborate with healthcare providers, community organisations, and other professionals to deliver effective support.
- Advocate for Clients: Ensure clients have access to essential services, such as housing, healthcare, and financial assistance.
- Maintain Records: Keep accurate and up-to-date records of assessments, interactions, and progress.
- Crisis Intervention: Provide immediate support during crisis situations, prioritising clients safety and well-being.
- Team Collaboration: Attend and contribute to multidisciplinary meetings and case discussions.
Qualifications and Skills- A recognised Bachelors or Masters degree in Social Work.
- Social Work England (SWE) registration.
- Experience working with adults in a social work or related setting is desirable.
- Strong communication, interpersonal, and organisational skills.
- Ability to work independently and collaboratively.
- Empathy, patience, and resilience when managing challenging situations.
